My 600-Lb Life: Why have gastric bypass surgery after losing a lot of weight on your own?

I'm a huge fan (and proponent) of TLC's reality series My 600-Lb Life, but there has always been one thing that bugged me about it. Dr. Nowzaradan always asks the potential gastric bypass surgery patients on the show to demonstrate that they are able to lose weigh ton their own before he will allow them to undergo the procedure. What I don't understand is when patients are able to lose a substantial amount of weight through diet and exercise at this stage, why is the option of not having surgery ever discussed?
